Written or Recorded Statement(s)
Comprehensive in-person interview with the
subject that focuses on all relevant and important issues relating to the
claim whether it be a motor vehicle accident, workers’ comp injury or
disability personal contact. The interview includes one recorded or
written statement, two hours travel and professional report documentation.
Claims Investigations
This is a complete investigation tailored
to fit the claim. Such investigation may include scene photos and
diagrams, vehicle inspections, measurements, witness canvasses, statements
and document retrieval.
Activity Check
This is used to determine the claimant’s
daily activities though interviewing neighbors lending insight into
subject’s usual activities, physical capacity and employment status as
related to workers’ comp injury, death benefits, disability or bodily
injury claims. A social makeup of the neighborhood, description and photos
of the residence and vehicle’s at the residence if available. Verification
of the claimant’s residence, telephone, social security number, and
registered vehicles at the claimant’s.
Medical Investigations
This investigation is used to determine
whether the claim is compensable and whether there was a preexisting
condition. To determine this we will examine the claimant’s medical
reports and bills from each of the claimant medical providers, including
doctors, hospital and clinic. A medical authorization should be obtained
for this type of investigation.
Standard Background Investigation
The Standard Background Investigation
provides an in-depth investigations into the claimant’s current and
previous residential history; public record check, which includes, Liens &
Judgments, Bankruptcy, Business Affiliations, Professional Licenses, Real
Property Ownership, State Driving Record, Asset Search (Vehicle(s), Boat,
etc) and a courthouse search of Criminal & Civil records (up to 1 county
are searched) covering the last 7 years within the current county where
the claimant resides.
Comprehensive Background Investigation
The Comprehensive Background Investigation provides an in-depth
investigations into the claimant’s current and previous residential
history; public record check, which includes, Liens & Judgments,
Bankruptcy, Business Affiliations, Professional Licenses, Real Property
Ownership, State Driving Record, Asset Search (Vehicle(s), Boat, etc)
Federal Criminal/Civil Records Search, Statewide Criminal Search (in
available states) , Criminal & Civil records (up to 2 counties are
searched) covering the last 7 years and Workers’ Compensation History
Check (signed release may be required).
Alive & Well Checks
This investigation is performed by CKI investigative team to verify a
claimant's current living and activity status. Our basic check involves a
brief telephonic interview with the claimant, verification of
employment/disability status, ss #, address, telephone and other available
information.
Accident Scene Investigations
This service includes photographs of the accident scene, video walk-thru
of the accident scene, vehicle(s) and a diagram of the accident scene.
Low Impact Accident Investigation
Scene, vehicles and interviews include photographs, video and a diagram of
the accident scene, as well as, photographs of accident vehicle(s) and
written or recorded interview with the drivers of the vehicles (2) in the
same geographical area.
Hospital / Pharmacy Canvass
This service is conducted within a 60 mile radius that surrounds your
claimant's listed addresses to determine if the claimant received any
medical treatment or obtained any prescription medication during the
months or years proceeding the reported injury or date of loss (12
hospitals or pharmacies will be checked with a signed release).
Locate Service / Skip Trace
With the use of extensive database searching and neighborhood canvassing
we will locate the whereabouts of your subject.
Advanced Locate Investigation
This type of specialized investigation involves an extensive field
investigation and additional database searches to locate your subject,
when other database services have failed.

Contestable Death Claims
Involves investigation designed to develop information relative to
the insurability of the deceased at the time of effective date of
coverage. Areas of investigation include medical, financial, occupational
